In order to qualify as a patient at CMCAC, you must be an Aiken County resident, (b) not covered by Medicare, Medicaid or Veterans benefits or any other medical or prescription drug insurance, (c) you cannot be a patient of another clinic,  (d) household income has to be at or below 200% of the Federal Poverty Guidelines,  (e) have a qualifying diagnosed chronic health condition and (f) be between the ages of 19-64. Anyone 18 years or older may participate in our R.I.S.E. (Real Independence Self-Empowerment) program. Make sure to let the staff know that you are wanting to schedule a R.I.S.E. assessment appointment.

The first step is to contact the clinic for a mini-screening and to schedule a screening appointment. An appointment can be scheduled after the screening process is completed and qualification is approved. Patient screenings are held on Mondays and Wednesdays, from 1 p.m. – 3 p.m. Please call 803-226-0630 to schedule an appointment. Office visits are scheduled for every 3-4 months or as frequently as necessary to stabilize a patient.

Office hours are Monday – Wednesday, 9:00 a.m. – 5:00 p.m., and Thursday, 9 a.m. – 3:00 p.m.

Submit Required Documentation:

  • South Carolina Driver’s License or Personal Identification Card from the Department of Motor Vehicle of South Carolina
  • Social Security Card
  • Past year’s income tax papers
  • Current pay stubs
  • Social Security Disability Benefit Statement
  • Food Stamp Benefit Statement
  • Unemployment Benefit Statement
  • Medicaid Denial Letter
